Liability Waiver and Parental Consent: SEN Martial Arts

Acknowledgement of Risk
I acknowledge that my child’s participation in this SEN martial arts programme involves physical activity and martial arts training. I understand and accept that there are inherent risks associated with such activities, including but not limited to the risk of injury.

I accept that activities may involve:
- Physical contact or impact with other participants.
- Physical contact with instructors or assistants (for demonstrations, support or safety)
- Physical exertion

Health & Fitness
- I am aware of my child’s current fitness level and any pre-existing medical conditions or injuries that may affect their participation. I have disclosed this information accurately in the registration form and accept responsibility for managing any such conditions.
- I understand that it is my responsibility to inform the instructor of any relevant SEN needs, medical conditions, or changes in health.
- I understand that participation is voluntary, and my child may withdraw from an activity if uncomfortable.

Behaviour & Intentional Harm
- A full risk assessment and safeguarding plan are in place to minimise risk.
- However, I acknowledge that in SEN group settings, there is a possibility of one child intentionally hurting another.
- I accept that Caged Steel Communities CIC and its instructors are not liable or insured for deliberate acts of harm by one participant towards another.
- I agree that I remain responsible for my child’s behaviour and accept that persistent unsafe or aggressive behaviour may result in removal from the session for the safety of all participants.

Liability Release
- I commit to ensuring my child follows all safety guidelines and instructions provided by the course instructors and staff, including the use of protective equipment where required.
- I acknowledge that the instructors and organisers of the programme are not responsible for injuries or accidents that may occur, except in cases of wilful misconduct or negligence.
- I accept that no physical activity can offer a complete guarantee of safety, and participation involves unavoidable risks.

Consent
By agreeing, I voluntarily consent to my child’s participation in the SEN Martial Arts & Personal Safety Programme, with full understanding of the risks, responsibilities and safeguards explained above.
